Visitation Help – ID Requirements
What can you use to identify yourself?
The following is required to verify your identity, anything not meeting these requirements will not be considered Valid.
For Adults (18+)
An ID card that:
- Contains
- Photo of Visitor
- Name
- When checking into the Public Lobby, must be an Active Name
- When submitting a BOP Request, must match a name provided with, either as your True Name or an Alternative Name
- Date of Birth (must match your account)
- Expiration date
- Is not expired
Examples include:
- MA state-issued Driver’s License or Photo ID
- Out-of-State Driver’s License or Photo ID
- Passport
- Military ID
- License-to-Carry
For Minors (under 18)
No Photo ID card is required, but the following must be presented:
- Birth Certificate (original or copy, watermarked with official seal)
- Accompanying Adult who must present
- A valid ID of their own
- Custody documentation upon the initial visit and again when the documentation expires
Documentation that proves custody
- If Biological Parent
- Adult’s ID must match a Name listed on Minor’s Birth Certificate
- If Adoptive Parent / Legal Guardian
- Custody documentation from an Official Court
- Adult’s ID must match Custodian named in Custody documentation
- Minor’s Birth Certificate must match Minor named in Custody documentation
- If DCF Personnel
- Official Letter stating DCF has custody of Minor
- Adult must present valid DCF identification
- Minor’s Birth Certificate must match Minor named in Custody documentation
- If non-Guardian Civilian
- Letter from the Parent giving consent for Visit
- Letter must be notarized
- Letter must contain start date of Authorization
- Letter must contain an expiration date of Authorization
- Date of Visit must not be after expiration date
- Parent’s Name must match a Name listed on Minor’s Birth Certificate
- Letter from the Parent giving consent for Visit
